Chronic Condition Management Plan

A Chronic Condition Management (CDM) Plan, formerly known as an Enhanced Primary Care (EPC) Plan, is a Medicare initiative that allows individuals with a chronic or long-term health condition to access allied health services such as physiotherapy or occupational therapy at a subsidised rate. This plan is developed by your General Practitioner (GP) and provides up to five rebated allied health sessions per calendar year to help manage your ongoing condition and improve your quality of life.

At Move Right Phsyiotherapy & Allied Health, we bulk bill patients under their CDM plan if they are a pensioner or hold a concession card. There is a gap fee of $25 per appointment, for patients without.

How to get started

  1. Visit your GP – Speak with your doctor about your ongoing condition and request a Chronic Disease Management (CDM) Plan referral for physiotherapy or occupational therapy.

  2. Book your appointment – Once your GP has completed the plan, contact our clinic to book your initial appointment.

  3. Bring your paperwork – Please bring a copy of your CDM referral form and any relevant medical information to your first session.

  4. Payment details – A $25 gap fee applies for clients who are not pension or health care (concession) card holders.
